The frame was warped and twisted because it wasn't shimmed or leveled during installation. The screws were overtightened, pulling the frame out of square. No exterior caulking, so moisture got in and accelerated the warping.
Removed the old door and frame completely. Inspected the opening (found some minor rot that we repaired). Installed a new quality storm door with proper shimming, leveling, and sealing. Added weatherstripping and adjusted the closer.
